People residing in the Hunter PIR Region living with severe and persistent mental illness & complex needs, and those who share their lives including families, carers, friends and social supports
The Hunter PIR Program is led by Hunter Primary Care in partnership with a Consortium of eight (8) other government and community managed organisations. The Consortium members include:

- Hunter New England Local Health District – Mental Health Services
Five (5) organisations have partnered with Hunter PIR to host Support Facilitator teams across the Hunter PIR Region.

- Benevolent Society – Singleton / Muswellbrook / Upper Hunter
- Neami National – Lake Macquarie
- RichmondPRA – Gloucester / Great Lakes / Greater Taree
- Wesley Mission – Newcastle / Port Stephens
